Michelle Trachtenberg was in poor health before she died at age 39 on Feb. 26.
Several sources spoke to Us Weekly about the “Gossip Girl” alum’s final days after she got a liver transplant, which may have contributed to her death.
“She was very open with those close to her about having the transplant,” a longtime friend of Trachtenberg’s said, “but she never went into detail.”
Trachtenberg had the transplant within the last year and “spent a prolonged period of time in the hospital” before she was released in early 2025, Us reported.
Per The Post sources, it’s believed her body may have rejected the organ and that Trachtenberg died of natural causes. Her death was ruled “undetermined” last week after her family objected to an autopsy.
“The last time I saw her, something felt off,” an industry insider told Us of Trachtenberg.
“You could sense something was going on. She was really frail and had been sick for a while,” they added.
According to Trachtenberg’s friend, the “Ice Princess” star had more health struggles going on.
“She had back issues, and then bone problems and she also fell a few times,” said the friend. “Whenever we spoke, there was something medically wrong.”
Meanwhile, a source close to the situation noted, “Michelle was thrown a lot of curveballs with her health, but she did her best not to let it get her down.”
Trachtenberg spent most of her final years of life out of the spotlight. Her final acting job was doing voice work in the 2024 documentary “Spyral.” Before that, she hosted a true-crime series in 2021 and appeared in two episodes of the “Gossip Girl” reboot in 2022.
Her longtime friend told Us the “Buffy the Vampire Slayer” actress “was more of a recluse” before she died.
“She achieved huge success early on but lost her footing later,” the friend shared. “I think she gave up on the fame aspect and didn’t want to be part of Hollywood anymore.”
The Post has reached out to Trachtenberg’s rep for comment.
The Post exclusively confirmed the news of Trachtenberg’s death last Wednesday.
Her mother found her around 8 a.m. at her luxury apartment in Manhattan’s Central Park South neighborhood, sources said.
“It is with great sadness [we] confirm that Michelle Trachtenberg has passed away,” Trachtenberg’s rep Gary Mantoosh confirmed in a statement to The Post. “The family requests privacy for their loss. There are no further details at this time.”
Her death is not deemed suspicious, per law enforcement.
